The Software Upgrade Nightmare

The finance platform , originally scheduled for a weekend, extended into Tuesday. Invoice processing was offline for 27 hours.
IT had tested thoroughly, but an unexpected database issue surfaced only under production-scale loads. The vendor's support line was saturated.
By mid-day Tuesday, operations ran manual reconciliation on spreadsheets while engineering applied hotfixes. Three customers missed payment and required goodwill credits.
The postmortem produced nine action items. The most important: a mandatory staged rollout plan for any system touching finance, plus a vendor-side stand-by channel for future upgrades.
